Student Services Liability Release

All guests seeking student salon and/or spa services must fill out a Liability Waiver form before commencement of service. ​​​Guests must also upload a copy of their Driver's License or other form of identification when submitting a Liability Form.

For client's under 18 years of age, a parent or guardians signature must be obtained prior to commencement of service.

Please note: Elite House of Beauty by Elite Education Training Center cannot perform services of any type of individuals under the age of 13.
